In Nov 2019 Will Smith posted a YouTube vlog called "I Vlogged My Colonoscopy" to raise awareness about getting checked for colon cancer. It was a scheduled procedure, not an emergency. Doctors found and removed a precancerous polyp and said to come back in 2-3 years. Clickbait sites later edited it to make it look serious.

Those are real pics of Will Smith with his son Jaden Smith at events. Jaden has had a few different looks over the years - shaved head with chains, and later with dreads.

So: no hospital emergency, just Will being open about health screenings. And yes, that's definitely him + Jaden in the bottom shots.
